Embolisatie van hypervasculaire wervelmetastasen

Trans-arteriële embolisatie sluit de bloedtoevoer naar sterk doorbloede wervelmetastasen af, meestal van niercel- of schildkliercarcinoom. Preoperatieve embolisatie vermindert het bloedverlies tijdens wervelchirurgie bij angiografisch hypervasculaire tumoren en kan als palliatieve behandeling pijn en tumorlast verminderen. Een meta-analyse toont een significant lager intraoperatief bloedverlies, maar het voordeel is vooral aangetoond bij tumoren die angiografisch hypervasculair zijn. Embolisatie wordt meestal binnen 24 tot 48 uur vóór de operatie uitgevoerd.
Wanneer geschikt
- Sterk doorbloede ( hypervasculaire ) wervelmetastase, vooral van niercel- of schildkliercarcinoom, vóór geplande chirurgie.
- Angiografisch bevestigde hypervascularisatie; hier is de winst in bloedverlies het grootst.
- Ook als palliatieve behandeling bij pijn of tumorlast wanneer chirurgie niet aan de orde is.
- Uitvoering meestal 24 tot 48 uur vóór de operatie.
- Beoordeling in een MDO met de wervelchirurg.
