Cementaugmentatie (vertebroplastiek / kyfoplastiek) bij wervelmetastasen
vertebroplastiek / kyfoplastiek · pijn en stabilisatie
Bij pijnlijke wervelmetastasen, myeloomlokalisaties of pathologische inzakkingsfracturen geeft percutane cementaugmentatie (vertebroplastiek of ballonkyfoplastiek) snelle en duurzame pijnvermindering en herstel van functie. In de gerandomiseerde CAFE-trial (2011) verbeterde ballonkyfoplastiek bij kankerpatiënten de rugfunctie, pijn en kwaliteit van leven binnen 1 maand duidelijk meer dan niet-chirurgische behandeling. Combinatie met radiofrequente ablatie voegt lokale tumorcontrole toe. De behandeling is aanvullend op, en geen vervanging van, radiotherapie en systemische therapie.
Wanneer geschikt
- Pijnlijke wervelmetastase, myeloomlokalisatie of pathologische inzakkingsfractuur die correspondeert met de klachten (bevestigd op MRI).
- Onvoldoende pijnrespons op of contra-indicatie voor conservatieve behandeling; pijn vooral mechanisch en axiaal .
- Bij voorkeur een intacte achterste wervelwand; bij aantasting zorgvuldige selectie wegens cementlekkage-risico .
- Geen ernstige epidurale compressie met myelopathie of instabiliteit die eerst chirurgie of decompressie vereist.
- Combinatie met RFA te overwegen voor lokale tumorcontrole; beoordeling in een MDO met radiotherapie en chirurgie.
