Trans-arteriële embolisatie bij tumorbloeding

Bij een bloedende tumor is embolisatie vaak de snelste manier om de bloeding te stoppen zonder een operatie. De cijfers per orgaan zijn consistent hoog: bij blaasbloeding 99,0% technisch en 80,9% klinisch succes met 5,5% majeure complicaties, bij gastro-intestinale bloeding 97,7% en 80%, en bij maligne hemoptoë 91,2% technisch succes, gelijk aan dat bij benigne oorzaken. Tegenover chirurgie voor refractaire bovenste-tractusbloeding ruilt embolisatie een hoger recidief (OR 2,44) voor duidelijk minder complicaties (OR 0,45), zonder verschil in mortaliteit. Eerlijk gezegd: de bloeding stopt, de tumor blijft, en de overleving wordt bepaald door de onderliggende ziekte.
Wanneer geschikt
- Actieve of recidiverende bloeding uit een tumor: hemoptoë bij longcarcinoom, hematurie bij blaas- of niertumor, gastro-intestinale bloeding of een geruptureerd hepatocellulair carcinoom.
- Bij hemoptoë is embolisatie geïndiceerd bij levensbedreigende of recidiverende bloeding waarbij pathologische arteriën worden gezien; bij maligniteit is het doel permanente occlusie van de abnormale circulatie distaal.
- Bij gastro-intestinale bloeding wanneer de bron endoscopisch niet behandelbaar of niet te vinden is.
- Bij hematurie na falende conservatieve behandeling; embolisatie is dan effectief en veilig als tijdelijke hemostase.
- Beoordeling met de behandelend specialist en, waar relevant, de chirurg; bij een geruptureerd HCC hoort de leverfunctie expliciet meegewogen te worden, want leverfalen is de belangrijkste doodsoorzaak.
