Echogeleide thermale ablatie bij papillair schildkliercarcinoom

Echogeleide thermale ablatie is bij laag-risico papillair schildkliercarcinoom opgeschoven van experiment naar erkende optie. Het internationale expertconsensus in Radiology beveelt haar aan als eerstelijnsoptie bij T1aN0M0 (sterke aanbeveling) en als alternatief bij T1bN0M0 wanneer de patiënt active surveillance en chirurgie afwijst of niet operabel is. In een cohort van 1613 patiënten verdween de tumor bij 85,3%, met lokale progressie bij 4,3%, complicaties bij 2,0% en majeure complicaties bij 0,4%; bij tien jaar follow-up (179 patiënten) was de ziektevrije overleving 93,9%. De sterkste vergelijkende winst zit niet bij primaire tumoren maar bij recidief kliermetastasen: tegenover heroperatie 2,8% tegen 7,0% permanente stembandzenuwuitval en 11,3% tegen 45,1% totale complicaties. Een gerandomiseerde vergelijking met chirurgie of active surveillance ontbreekt.
Wanneer geschikt
- T1aN0M0 papillair carcinoom (tot 1 cm): ablatie is een van de eerstelijnsopties (sterke aanbeveling, hoge bewijskracht).
- T1bN0M0 (1 tot 2 cm): alternatief wanneer de patiënt active surveillance en chirurgie afwijst of niet operabel is (matige aanbeveling). Let op: complete verdwijning was hier 71,4% tegen 99,4% bij T1a.
- Afstand van ten minste 0,5 cm tot nervus recurrens, trachea en oesofagus; met hydrodissectie is 5 mm of meer in de paratracheale ruimte een veilige afstand.
- Recidief of restziekte in de hals en radioactief-jodium-refractaire kliermetastasen , vooral na eerdere halsklierdissectie, bij chirurgisch risico of bij minder dan 4 aangedane klieren.
- Intraprocedurele beoordeling van de stembandbeweging op echo; beoordeling in een multidisciplinair team met interventieradiologische expertise. Contra-indicaties zijn agressieve cytologische subtypen, extrathyreoïdale uitbreiding, multipele haarden en klier- of afstandsmetastasen.
