Percutane ablatie van niercelcarcinoom

Bij kleine niertumoren (cT1a, ≤4 cm) is percutane thermale ablatie (cryoablatie, RFA of microwave) een nefronsparende, minimaal invasieve behandeling met hoge lokale controle en behoud van nierfunctie. In grote observationele cohorten zijn de oncologische uitkomsten voor cT1a vergelijkbaar met partiële nefrectomie, met minder morbiditeit. Richtlijnen (EAU, AUA) bevelen ablatie vooral aan bij oudere of comorbide patiënten, een solitaire nier of chronische nierschade. Een percutane biopsie vooraf voor histologische diagnose is standaard.
Wanneer geschikt
- Kleine niertumor cT1a (≤4 cm) , met het sterkste bewijs; exofytische, goed bereikbare laesies zijn het meest geschikt.
- Vooral bij oudere of comorbide patiënten, een solitaire nier, bilaterale tumoren of chronische nierschade, waar nefronsparen belangrijk is.
- Percutane biopsie vooraf (niet gelijktijdig) voor histologische diagnose is standaard.
- RFA bij voorkeur tot ongeveer 3 cm, cryoablatie tot ongeveer 4 cm; centrale of hilaire ligging vraagt zorgvuldige selectie.
- Beoordeling in een uro-oncologisch MDO, met partiële nefrectomie en active surveillance als alternatieven.
