Transarteriële embolisatie bij NET-levermetastasen

Bij niet-resectabele, leverdominante NET-levermetastasen is transarteriële embolisatie een gevestigde behandeling voor zowel tumorcontrole als verlichting van hormonale symptomen. In de gerandomiseerde RETNET-trial (2025) gaven bland embolisatie (TAE) en lipiodol-chemo-embolisatie (cTACE) een gelijke lever-progressievrije overleving en vergelijkbare symptoomcontrole; de DEB-TACE-arm werd vroegtijdig gestopt wegens hepatobiliaire toxiciteit. Radio-embolisatie (Y-90) is een alternatief, maar bij somatostatinereceptor-positieve ziekte die later voor PRRT in aanmerking komt gaat de voorkeur vaak uit naar TAE of TACE vanwege langetermijn-levertoxiciteit.
Wanneer geschikt
- Niet-resectabele, leverdominante of lever-only metastasen, meestal van goed gedifferentieerde (G1/G2) NET.
- Progressieve ziekte onder systemische therapie, of hormonale symptomen (carcinoïdsyndroom) die onvoldoende reageren op somatostatine-analogen.
- Voldoende leverfunctie en een patente vena portae; embolisatie belast het leverparenchym.
- Zowel functionele als niet-functionele NET komen in aanmerking.
- Beoordeling in een NET-MDO, met de sequentie ten opzichte van PRRT en systemische therapie vooraf bepaald.
