Percutane ablatie van longtumoren
RFA / microwave / cryoablatie · NSCLC en longmetastasen
Percutane thermale ablatie (RFA, microwave of cryoablatie) is een minimaal invasieve, longsparende behandeling voor kleine, niet-resectabele longtumoren: inoperabel vroeg-stadium NSCLC en longmetastasen (vooral colorectaal). In een meta-analyse van longmetastase-ablatie was de 1-jaars lokale controle 91% en de mediane overleving 5,1 jaar, met een majeure-complicatiekans van 1% en zonder periprocedurele mortaliteit (Nguyenhuy et al., CVIR 2022). Cryoablatie gaf in de ECLIPSE-studie duurzame lokale controle en een gunstige 5-jaarsoverleving. Ablatie is herhaalbaar en een alternatief voor of aanvulling op SBRT en chirurgie.
Wanneer geschikt
- Klein (meestal ≤3 cm ), niet-resectabel: inoperabel vroeg-stadium NSCLC of beperkte longmetastasen.
- Oligometastatische ziekte, vaak colorectaal, met een gecontroleerde primaire tumor.
- Voldoende afstand tot centrale structuren (grote vaten, hoofdbronchus); perifere laesies zijn het meest geschikt.
- Behoud van longfunctie belangrijk; ablatie is herhaalbaar en longsparend.
- Beoordeling in een MDO, met SBRT en chirurgie als alternatieven.
