Trans-arteriële chemo-embolisatie (TACE) bij HCC

TACE is de standaardbehandeling voor intermediair (BCLC B) hepatocellulair carcinoom: niet-resectabele, multinodulaire tumoren zonder vasculaire invasie of extrahepatische spreiding, met behouden leverfunctie. Gerandomiseerde trials en meta-analyses toonden een overlevingswinst van TACE ten opzichte van ondersteunende zorg (Llovet et al., Lancet 2002). Drug-eluting-bead-TACE geeft vergelijkbare effectiviteit met een betere verdraagbaarheid dan conventionele TACE (PRECISION V; Lammer et al., CVIR 2010). TACE kan ook als brug naar transplantatie of gecombineerd met ablatie worden ingezet.

Wanneer geschikt

• Intermediair HCC (BCLC B): niet-resectabele, multinodulaire ziekte zonder macrovasculaire invasie of extrahepatische spreiding .
• Behouden leverfunctie (Child-Pugh A of goede B) en een patente vena portae.
• Superselectieve toediening waar mogelijk, om gezond leverparenchym te sparen.
• Ook als brug naar transplantatie of downstaging, of gecombineerd met ablatie bij grotere laesies.
• Beoordeling in een lever-MDO; TACE 'on demand' herhalen bij respons en stoppen bij TACE-refractoriteit.
