Thermale ablatie bij hepatocellulair carcinoom

Bij vroeg hepatocellulair carcinoom (BCLC 0/A) met een of enkele kleine tumoren (tot ongeveer 3 cm) is thermale ablatie (RFA of microwave) een curatieve behandeling, met overleving vergelijkbaar met chirurgische resectie bij kleine tumoren en behoud van leverfunctie. In de gerandomiseerde SURF-trial waren recidiefvrije en totale overleving na RFA niet significant verschillend van resectie. Microwave-ablatie is non-inferieur aan RFA. Ablatie is de voorkeursbehandeling bij niet-resectabele vroege HCC en bij patiënten met beperkte leverreserve, en kan als brug naar transplantatie dienen.
Wanneer geschikt
- Vroeg HCC (BCLC 0/A): één tumor of tot drie tumoren ≤3 cm , met behouden leverfunctie (Child-Pugh A of goede B).
- Sterkste resultaten bij tumoren ≤2 tot 3 cm ; boven 3 cm neemt lokaal recidief toe.
- Lokalisatie op veilige afstand van grote vaten (heat-sink) en galwegen; anders IRE of een gecombineerde aanpak overwegen.
- Ook inzetbaar als brug of downstaging naar levertransplantatie.
- Beoordeling in een lever-MDO, met resectie en transplantatie als alternatieven.
