Elektrochemotherapie (ECT) combineert een laaggedoseerd chemotherapeuticum (bleomycine of cisplatine) met korte elektrische pulsen die de celmembraan doorlaatbaar maken, waardoor het middel lokaal veel effectiever wordt. Het is een gevestigde, minimaal invasieve palliatieve behandeling voor cutane en subcutane metastasen van uiteenlopende tumoren, waaronder melanoom en thoraxwand-recidief van mammacarcinoom. In de ESOPE-studie was de objectieve respons ongeveer 85% met een complete respons rond 74% (Marty et al., EJC Suppl 2006). ECT geeft snelle lokale controle, pijnverlichting en verbetering van ulcererende of bloedende laesies, met een gunstig veiligheidsprofiel.
Wanneer geschikt
- Cutane of subcutane metastasen of lokaal recidief (bijv. melanoom, thoraxwand-recidief van mammacarcinoom, plaveiselcel- en basaalcelcarcinoom).
- Beperkt aantal, goed bereikbare oppervlakkige laesies; ook bruikbaar bij bloedende of ulcererende laesies.
- Palliatief doel: lokale controle, pijn- en symptoomverlichting en wondverbetering.
- Herhaalbaar en te combineren met systemische of immuuntherapie.
- Diepe (viscerale, bot-)laesies zijn mogelijk met speciale elektroden, maar nog grotendeels experimenteel.
