Elektrochemotherapie (ECT) bij radiotherapie-resistente epidurale wervelmetastasen

Bij metastatische epidurale ruggenmergcompressie (MESCC) die na radiotherapie progressief blijft, is er lang geen goede optie geweest naast chirurgische decompressie. Elektrochemotherapie combineert bleomycine met elektrische pulsen via transpediculair geplaatste elektroden en behandelt zo de epidurale tumorcomponent. In de grootste prospectieve serie (40 patiënten) was de objectieve respons 77% op 1 maand, daalde de pijnscore van NRS 7,0 naar 1,0 en had 55% neurologische verbetering (Deschamps et al., Eur J Cancer 2023). Daar staat een reëel neurologisch risico tegenover: paraplegie trad op bij 7,5%. ECT is hier een redmiddel na falende radiotherapie, geen eerstelijnsbehandeling, en hoort thuis in ervaren centra en bij voorkeur in studieverband.
Wanneer geschikt
- Radiotherapie-resistente epidurale spinale compressie: progressie of pijnrecidief na eerdere bestraling van dezelfde wervel.
- Geen effectieve systemische opties meer, of onvoldoende tijd om op systemische respons te wachten.
- Patiënt niet geschikt voor of niet gebaat bij chirurgische decompressie en stabilisatie.
- Transpediculaire toegang mogelijk met elektrodeplaatsing in de aangedane en meestal de aangrenzende wervel, onder beeldgeleiding.
- Beoordeling in een MDO met wervelchirurgie en radiotherapie; niet bij acute, snel progressieve myelopathie die directe decompressie vraagt.
