Thermale ablatie bij colorectale levermetastasen

Bij kleine (≤3 cm) colorectale levermetastasen geeft thermale ablatie een totale overleving die non-inferieur is aan chirurgische resectie, met minder complicaties, lagere mortaliteit en een kortere opnameduur (COLLISION, fase 3 RCT, 2025; ondersteund door MAVERRIC, 2023). Bij irresectabele, lever-beperkte ziekte verbetert ablatie bovenop chemotherapie de langetermijnoverleving ten opzichte van chemotherapie alleen (CLOCC, RCT). Een adequate ablatiemarge (≥5 tot 10 mm) is bepalend voor de lokale controle.
Wanneer geschikt
- Metastasegrootte ≤3 cm , met het sterkste bewijs; de lokale controle benadert die van resectie. Dit is de kern van de indicatie.
- Tot ongeveer 10 laesies, lever-beperkte ziekte, beoordeeld in een multidisciplinair team.
- Lokalisatie die een veilige marge (≥5 tot 10 mm) toelaat, weg van grote vaten (heat-sink) en centrale galwegen.
- Zowel resectabele als irresectabele patiënten komen in aanmerking; de indicatie is niet beperkt tot irresectabele ziekte.
- Intermediaire grootte (3 tot 5 cm): een actief onderzoeksgebied (zie COLLISION-XL), buiten studieverband selectief in te zetten.
