Galwegdrainage en endobiliaire RFA bij (peri)hilair cholangiocarcinoom

Bij niet-resectabel (peri)hilair of extrahepatisch cholangiocarcinoom verlicht percutane galwegdrainage de maligne galwegobstructie (geelzucht, cholangitis) en herstelt het de leverfunctie. Endobiliaire (intraductale) radiofrequente ablatie verlengt de doorgankelijkheid van de stent en geeft in meerdere gerandomiseerde studies een overlevingsvoordeel. Een meta-analyse van RCT's toonde zowel een langere stent-doorgankelijkheid als een overlevingswinst met RFA plus stent versus stent alleen (Cancers 2024). Drainage en ablatie zijn palliatief en worden gecombineerd met systemische therapie.
Wanneer geschikt
- Niet-resectabel (peri)hilair of extrahepatisch CCA met maligne galwegobstructie (geelzucht, cholangitis of leverfunctiestoornis).
- Percutane transhepatische route wanneer endoscopische drainage niet lukt of ontoereikend is, vaak bij hoge (hilaire) obstructie.
- Endobiliaire RFA te overwegen om tumoringroei te behandelen en de stent-doorgankelijkheid te verlengen.
- Doel is meestal palliatief: symptoomverlichting en het mogelijk maken van systemische therapie.
- Beoordeling in een hepatobiliair MDO, met endoscopische drainage als eerste keuze waar mogelijk.
