Thermale ablatie bij intrahepatisch cholangiocarcinoom

Bij klein, niet-resectabel intrahepatisch cholangiocarcinoom (iCCA), meestal ≤3 cm, biedt thermale ablatie (RFA of microwave) lokale controle en overleving als alternatief voor resectie bij niet-operabele patiënten. In een meta-analyse van 917 patiënten was de technische effectiviteit 91,9% en waren de 1-, 3- en 5-jaars overleving 82%, 42% en 29%, met 5,7% majeure complicaties (Kim et al., Eur Radiol 2022). De resultaten zijn het best bij tumoren ≤3 cm; grotere en multifocale tumoren voorspellen een slechtere uitkomst. Ablatie wordt ingezet naast systemische therapie en als lokale behandeling van recidief.
Wanneer geschikt
- Klein, niet-resectabel iCCA ≤3 cm , met het sterkste bewijs; exofytische, bereikbare laesies zijn het meest geschikt.
- Niet-operabele patiënten of lokaal recidief na eerdere resectie.
- Veilige marge tot grote vaten en galwegen; anders IRE of een andere aanpak overwegen.
- Uitsluiten van uitgebreide multifocale of extrahepatische ziekte.
- Beoordeling in een hepatobiliair MDO, naast resectie en systemische therapie.
