Cryoablatie van vroeg mammacarcinoom

Bij klein, vroeg, laag-risico mammacarcinoom is percutane cryoablatie een opkomende, minimaal invasieve en borstsparende behandeling zonder chirurgische excisie. In de ICE3-trial (194 patiënten, tumoren ≤1,5 cm, ER-positief/HER2-negatief) was de 5-jaars ipsilaterale recidiefvrije overleving 96,3% (Fine et al., Ann Surg Oncol 2024). De FROST-trial bevestigt gunstige langetermijnuitkomsten. Cryoablatie is nog geen standaardbehandeling; ze wordt vooral onderzocht bij oudere patiënten of als borstsparend alternatief in studieverband, in combinatie met endocriene therapie en meestal radiotherapie.
Wanneer geschikt
- Klein (meestal ≤1,5 cm ), unifocaal, laag-risico invasief mammacarcinoom (vaak ER-positief/HER2-negatief), goed zichtbaar op echografie.
- Vooral bij oudere of niet-operabele patiënten, of als borstsparend alternatief in studieverband.
- Bevestigde histologie en beeldvorming; uitsluiten van uitgebreide DCIS of multifocaliteit.
- Combinatie met endocriene therapie en meestal radiotherapie volgens de standaard.
- Beoordeling in een mamma-MDO; cryoablatie is buiten studies nog geen standaard.
